The Patient Care Record is a collection of documents that provides the details in which a patient visited, or sought treatment, and received care, or a referral for care from a health care facility. These records maintained are confidential. Patient Care Records are classified as Continuity Care Document (CCD), Consolidated Clinical Document Architecture (C-CDA), and Continuity Care Record (CCR).
