Recording Refusal Dose for Adult

In a case when the patient refuses to take any particular immunization, or vaccines are out of stock in the practice, or the patient’s immunity is good, then you can record the refusal details for such patients under the Document Refusal tab.

 

To record Document Refusal dose:

1.In the tabular format shown in the Adult chart, immunization categories are displayed in the far left columns, and the dose numbers are displayed row-wise respective to each immunization category.
2.To record the refusal dose for an appropriate vaccine, click in the corresponding box.
3.The Immunization screen appears with the Administer Vaccine tab selected.
4.Click the Document Refusal tab.
5.In the Document Refusal tab, record the following details:
Category: This field is prepopulated and will remain in the disabled mode.
Vaccine: Select the refused vaccine from the drop-down list. This is a mandatory field.
Refusal Date: The date and time are auto-populated. If required, change the date and time on which the dose was refused. This is a mandatory field.
Reason: Select the refusal reason from the drop-down list. This is a mandatory field.
Provider: Select the provider from the drop-down list. This is a mandatory field. The provider name is auto-populated. If required, change the provider from the drop-down list. This is a mandatory field.
6.Click Save to save the details.

Note: The Refused Dose is not displayed on the Encounter note. It will be displayed only on the Immunization Report
